direct and indirect care to patients during wellness visits,
addressing psychosocial, physical, and preventive health needs.
Communicates with patients, families, and the care team to
coordinate services before the provider’s face-to-face visit.
Follows regulatory guidelines and clinic policies under the
supervision of a nurse practitioner or physician. To perform this

discretion. Education Required - Graduate of an accredited school
of nursing. Work Experience Required – 3 years of nursing
experience Preferred – Experience in home health or clinic setting
Certifications Required – Current registered nurse (RN) license in
state of practice. Current Basic Life Support (BLS) certification
from the American Heart Association. Knowledge Skills and Abilities
(KSAs) - Good organizational and time management skills and ability
to be self-directed and demonstrate good judgement. - Good
interpersonal skills. - Ability to work with and maintain
confidential information. - Ability to work in a fast-paced
environment with minimal supervision. Job Duties - Provides direct
patient care, evaluates outcomes, assesses risk factors and patient
needs, and consults with other health team members as required
under the supervision of a NP and/or physician. - Determines what
preventive services the patient qualifies for utilizing the
appropriate Medicare/Medicare Advantage portal tools. Educates the
patient and family regarding these services and any available
resources. - Provides continuity by ensuring accurate appointment
type set-up, prepping the paperwork for visit, and performing all
aspects of the wellness visit per policy. - Ensures the provider
has information documented for correct and complete billing of
services rendered after face-to-face portion of the encounter. May
also assist provider with procedures as needed, both technical and
manual, under the supervision of a NP and/or physician. - Focuses
on identifying and addressing chronic disease diagnoses,
particularly Hierarchical Condition Categories (HCC), to support
effective risk management and ensure comprehensive patient care. -
Adapts behavior to the specific patient population, including but
not limited to respect for privacy, method of introduction to the
patient, adapting explanation of services or procedures to be
performed, requesting permissions and communication style. -

entails pushing and/or pulling of arm or leg controls; and/or (3)
when the job requires working at a production rate pace entailing
the constant pushing and/or pulling of materials even though the
weight of those materials is negligible. NOTE: The constant stress
and strain of maintaining a production rate pace, especially in an
industrial setting, can be and is physically demanding of a worker
even though the amount of force exerted is negligible. Duties
performed routinely require exposure to blood, body fluid and
tissue. The incumbent works in a patient care area; works in an
area where patients enter; works directly with patients; and/or
works with specimens that could contain communicable diseases. The
incumbent has an occupational risk for exposure to all communicable
diseases. Because the incumbent works within a healthcare setting,
there may be occupational risk for exposure to hazardous
medications or hazardous waste within the environment through
receipt, transport, storage, preparation, dispensing,
administration, cleaning and/or disposal of contaminated waste. The
risk level of exposure may increase depending on the essential job
