The Care Manager plays a vital role within our interdisciplinary team, focusing on assessing, planning, advocating, and coordinating patient care from the moment of admission through to discharge. This position ensures a smooth transition after hospitalization, aligning with our organization's mission, values, and service standards. Our Care Management model emphasizes effective transition planning and oversight of length of stay, all aimed at enhancing patient experience, safety, and quality of care by utilizing performance metrics and implementing best practices. Position Overview: Required Education: • Graduate from an accredited Nursing School is essential. Experience: • Preferred 2-3 years in an acute care hospital environment. • Experience in Care Management or Utilization Management is a plus. Licensure: • A current license in the state of employment is mandatory. Certifications: • Basic Life Support (BLS) certification is required upon hire or within 90 days of starting. • Certification in Case Management is preferred. Why Choose Us?
