(Non-Exempt) Summary: The Nurse Navigator plays a pivotal role in
leading and coordinating patient care across both inpatient and
outpatient environments, ensuring that treatment is delivered
safely, effectively, and promptly. This position involves
collaboration with patients, families, healthcare providers, and
interdisciplinary teams to manage care plans, discharge processes,
and utilization management, all while upholding professional
standards. A registered nursing license and clinical expertise are

us! The Nurse Navigator serves as a leader, offering support,
mentorship, and guidance to those involved in direct patient care.
This role entails assessing individual patient care needs and
making recommendations based on the severity of illness, required
service intensity, and skill level. The Nurse Navigator ensures
that patients receive timely, safe, and high-quality care,
coordinating care plans with bedside nurses. They establish
expectations for professional conduct and act as a resource for the
patient care unit. Additionally, the Nurse Navigator holds both
direct and indirect care providers accountable for their
professional duties and compliance with regulatory standards. This
position is responsible for managing both inpatient and outpatient
populations, facilitating an effective new care model that
emphasizes quality patient care, safety, and financial
considerations. The role promotes the integration of a seamless
care model, coordinating discharge planning for post-acute
services, managing length of stay (LOS), and overseeing utilization
management. Collaboration is key, as evidenced by performance
metrics, to optimize outcomes and implement best practices.
Responsibilities include pre-operative education, daily patient
rounds during hospitalization, discharge planning, and follow-up

Friday Overview: The Nurse Navigator is a nursing specialist who

interdisciplinary teams, and support staff. Qualifications:
Experience: Minimum of 2 years in a related field Required
Education: Graduate from an accredited nursing program Preferred
Education: Bachelor of Science in Nursing Certifications: Must hold
a valid RN license in the state of practice. Basic Life Support
