Job Description Job Description NURSE PRACTITIONER Liberty Cares With Compassion At Liberty Hospice we understand the unique needs of our patients and families facing terminal illness. That is why Liberty Hospice provides our hospice patients with state-of-the-art care and pain management services, delivered by our specially trained staff with emphasis on strength, dignity and compassion. We are currently seeking an experienced: HOSPICE & PALLIATIVE CARE NURSE PRACTITIONER Per Diem (Covering Onslow and surrounding counties) Job Description: Implements palliative care program requirements Completes medical visit with patients per guidelines and coordinate care with Palliative Care team as clinically indicated Conduct face to face encounter to provide re-certification information to Medical Director/Associate Medical Director for Hospice Patients Provide clinical findings of that face to face encounter Primary care physician, and/or supervising Medical Director/Associate Medical Director Communicate with patients/families at all levels. Communicate with all agency personnel Job Requirements: Master’s degree in Nursing required, with current licensure in the state of practice Certification as an Adult or Family Nurse Practitioner Experience in Hospice or Palliative Care nursing preferred Knowledge of the framework, function, and organization of a hospice care program.
