Perform wound care procedures such as debridement, dressing changes, and wound vac applications. Monitor and document patient progress, adjusting care plans as needed. Educate staff, patients, and families on wound care techniques and best practices to prevent infection and promote healing. Collaborate with physicians, nurses, and other interdisciplinary team members to ensure coordinated care. Maintain accurate documentation of wound care assessments, treatments, and outcomes in the patient's medical record. Ensure compliance with federal, state, and facility regulations related to wound care. Participate in quality assurance and performance improvement (QAPI) programs related to wound care management. Provide leadership and guidance to nursing staff on wound care best practices. Qualifications: Valid and active RN or LPN/LVN license in PA Wound Care Certification (WCC, CWCN, or equivalent) is preferred. 2 years of experience in wound care, preferably in a long-term care or skilled nursing facility setting preferred Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Detail-oriented with strong organizational skills.
