Clinical Manager – Homecare Position Summary: Responsible for the oversight and supervision of the clinical homecare staff. Ensures patient care is delivered in compliance with Compass Regional Hospice (Compass) policy and procedures as well as state and federal regulations and industry standards. Reports to: Director of Clinical Services Supervisory Responsibilities: This position is responsible for the direct supervision of the RN Case Managers, Admissions Nurses, Clinical Liaison, CNA Homecare Manager, and the overall function of the home care program. Responsibilities & Functions: Coordination of direct and indirect patient services provided by all clinical homecare staff. Provides guidance and counseling to all clinical homecare staff to assist in improving all aspects of hospice care services. Assists clinicians in setting priorities in developing and establishing immediate and long-term therapeutic plans of care. Assist in the preparation of the annual budget for homecare. Identifies needs for clinical staff education. Maintains and delegates the maintenance of electronic clinical records, statistics, reports, and records for purposes of evaluation and reporting of organization activities. Assures proper maintenance of medical records as prescribed and in compliance with local, state, and federal laws. Maintains or delegates the maintenance of adequate and appropriate inventory supplies for the provision of patient services. Promotes hospice care in the healthcare community. Maintains good standing in the medical community. Functions as a role model in area of clinical specialty, demonstrating competency in clinical judgment, technical skills, and appropriate knowledge base to lead and guide staff. Demonstrates clinical expertise in the execution of the nursing process and directs staff in its application to promote safe, effective, and appropriate care. Conducts patient visits to offer support and expertise to the nursing staff, patients, and families. Serves as a resource, ensuring that patient problems are addressed and resolved in a timely manner. Ensures staff understand and exhibit the hospice philosophy through education and role modeling. Provides daily oversight to ensure that homecare admissions are processed in an efficient and effective manner, ensuring timely patient response and services. Ensures that all admissions activities and processes are functioning in a manner that will optimize growth while in compliance with all clinical operation and regulatory requirements. Participates in and supports continuous process improvements. Works in conjunction with the medical director(s) and hospice CRNP(s) in the provision of effective symptom management of all patients. Provides education to patients and families specific to the hospice benefit, end of life care and symptom management. Initiates communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family. Interacts with appropriate members of Compass staff to ensure patient changes are managed in a timely manner ex: changes in patient level of care, contemplated discharges, and patient hospitalizations. Leads and facilitates the IDG meeting on a regular basis. Offers oversight of medication management in the homecare setting to ensure medications and medication costs are managed efficiently by homecare case managers. Offers oversight to the homecare clinical staff to ensure patient documentation reflects appropriate diagnosis codes and up to date information in accordance with legal and regulatory standards. May function as an admitting nurse or case manager when needed. Maintains compliance with Medicare Regulations and Community Health Accreditation Program (CHAP) as appropriate. Demonstrate familiarity with policy and procedures of the agency and rules and regulations of State and Federal bodies. Interviews, hires, coordinates training, evaluations, learning plans and performance improvement of homecare staff. Provides regularly scheduled 1:1’s with staff to provide space for open communication, feedback, personal development, and guidance, to ensure success in their roles. Responsible and accountable for own nursing practice, continuing education, and professional development. Maintains the confidentiality of employees, patient/client, and agency information. Other duties as assigned. Qualifications: BSN or master’s in nursing, Healthcare Administration, or related field, preferred. Possession of a current license to practice as a registered professional nurse in the State of Maryland. Minimum of two years of hospice nursing experience with management experience, preferred. Ability to work independently with minimal supervision. Knowledge of Medicare, Medicaid, private insurance and other community programs and resources. Demonstrated ability to collaborate effectively with an inter-disciplinary team. Excellent customer service philosophy and skills required. Demonstrated organizational and time management skills. Strong computer skills, to include but not limited to Electronic Medical Records software applications, Microsoft Word and Outlook required. Strong oral and written communication skills. Requirements: Complete hospice orientation. Support the hospice concept of care. Be flexible and able to work on several projects simultaneously. Ability to cope with family emotional stress. Be able to work harmoniously with diverse types of individuals and groups. Advances knowledge and skills; attends in-services and workshops as appropriate. An ability to cope with the stress of continual loss. Possesses physical ability to perform job-related duties which may require lifting, standing, bending, transferring, stooping, stretching, walking, pushing, partial or complete assistance with activities of daily living without assistance from another health care worker or significant other. Must be able to hear, process directions, and requests from doctors, employees, patients, and other individuals. Must be able to drive to visit patients’ homes for supervision and patient care. Must have reliable transportation. Must complete Hospice and Palliative Care Certification (HPCC) within 24 months of hire date. Willingness to obtain or possess Delegating Nurse Certification, if requested. Schedule: Monday – Friday 8:00am – 4:30pm – On-site Primarily office-based with periodic travel to patient homes or facilities Must be available for rotating evening and weekend administrative on-call support. FLSA Status: This is a full-time position, classified as exempt under the Fair Labor Standards Act.
