Encompass Health Corporation, a leader in facility-based and home-based patient care, is seeking an experienced and dedicated Case Manager II to join our comprehensive healthcare team. This role is pivotal in facilitating and ensuring the delivery of high-quality care through both our inpatient and outpatient services. The Case Manager II will be responsible for assessing patient needs, developing care plans, coordinating with healthcare providers, and supporting patients and their families. This position demands a compassionate approach to patient interaction, strict adherence to healthcare regulations, and an aptitude for efficiently managing patient care processes. Assessing patient care requirements and creating detailed nursing care plans. Coordinating with physicians, nurses, and other healthcare professionals to ensure effective execution of care plans. Monitoring patient progress and making adjustments to care plans as required. Acting as a liaison between patients, families, and multidisciplinary teams to facilitate communication and ensure compliance with healthcare standards. Maintaining robust documentation for all case management activities. Providing education and support to patients and their families regarding healthcare processes and procedures. Strong interpersonal and communication skills. Ability to work independently and collaboratively. Proficient in using healthcare information systems. Knowledge of healthcare laws, regulations, and standards. Strong organizational and problem-solving skills. Medical Insurance Dental Insurance Vision Insurance 401k Certified Case Manager (CCM) preferred. Valid nursing or other relevant healthcare professional license.
