Job Description:
At Encompass Health Corporation, we are dedicated to providing

rehabilitation hospitals, home health agencies, and hospice
agencies. The role of Case Manager II is critical to our mission of
encompassing better health and personalized care for patients
during their recovery journey. As a Case Manager II, you will
oversee assessing, planning, and coordinating the ongoing care and
support for assigned patients. You will work closely with
healthcare professionals, families, and insurance providers to
ensure that each patient receives tailored, high-quality care.
Conduct comprehensive patient assessments to determine necessary
care and recovery plans. Collaborate with medical, nursing, and
administrative staff to ensure coordinated and comprehensive
patient care. Develop care plans based on patient needs and medical
advice, adjusting plans as circumstances change. Monitor patient
progress and outcomes, revising care plans as necessary. Advocate
for patients' needs and provide resources and support for families.
Liaise with insurance providers to secure necessary approvals and
verify patient coverage. Document all patient interactions,
treatments, and outcomes accurately and comprehensively. Strong
organizational and communication skills Ability to multitask and
manage time effectively Knowledge of healthcare regulations and
policies Proficiency with medical software and charting Empathy and
interpersonal skills Medical Insurance Dental Insurance Vision
Insurance 401k Certified Case Manager (CCM) credential is
preferred. State-issued Nursing or Social Work license as
applicable.
