Job Description:

standard of patient care. We are seeking a dedicated Case Manager
II to join our integrated healthcare team. This role is pivotal in
coordinating patient-centric rehabilitation services, bridging the
gap between the inpatient environment and home-based care. As a
Case Manager II, you will develop comprehensive care plans, monitor
patient progress, and ensure seamless communication among patients,
families, and healthcare teams to optimize recovery and facilitate
independence. Conduct comprehensive patient assessments to
determine the need for medical services and appropriate care
setting. Develop, implement, and revise individualized care plans
in collaboration with healthcare providers, patients, and their
families. Facilitate multi-disciplinary team meetings to review
treatment goals, progress and modify care plans accordingly.
Provide patient and family education on management of illnesses and
coping with lifestyle changes. Monitor patient progress and
coordinate with healthcare providers to ensure timely delivery of
diagnostic and treatment services. Maintain accurate patient
records, ensure compliance with federal and state regulations
concerning healthcare services. Assist patients and families in
understanding their insurance benefits and billing procedures.
Strong interpersonal and communication skills Ability to manage
multiple cases efficiently Proficient in healthcare databases and
electronic health record (EHR) systems Excellent organizational and
decision-making skills Medical Insurance Dental Insurance Vision
Insurance 401k Certified Case Manager (CCM) preferred Licensed
Registered Nurse (RN) or Licensed Social Worker (LSW) is a plus
